Salary: South African Rand (ZAR) Job Responsibilities Assess and evaluate patients' physical, emotional, and social needs to create comprehensive care plans. Coordinate with healthcare providers, patients, and families to ensure seamless communication and care delivery. Monitor patient progress and modify care plans as needed to achieve health goals.
