This position is responsible for implementing patient discharge plans, and supporting coordination of care activities under the direction of a registered nurse and/or social worker. The incumbent interacts with representatives from insurance companies, health maintenance organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies.
