I. SPECIFIC RESPONSIBILITIES
Supervision:
• Supervise and support Care Managers through regular individual and team supervision
• Oversee caseloads to ensure timely documentation and compliance with regulatory requirements
• Review and maintain electronic health records (EHR) for accuracy and completeness
• Provide coaching, performance feedback, and professional development support
• Assist with audit preparation and ensure program readiness Key Performance Indicators (KPIs,
documentation, reporting)
• Participate in field visits, case conferences, and interdisciplinary team meetings (IDT)
• Manage staff schedules and coverage; provide direct services as needed
• Collaborate with internal teams and community partners to support client outcomes
Training & Development:
• Schedule and track required trainings (DOH, Compliance, HIPAA, Health Homes, Relias,
