About the Role

The Care Manager leads multidisciplinary care delivery for adults and families with mental health needs, working autonomously with Tangata Whaiora, whānau, and other service providers to plan and coordinate interventions, complete assessments and monitor outcomes.

Key Responsibilities

• Conduct comprehensive mental health assessments and develop individualized treatment plans.
