Description
The Semel Institute for Neuroscience and Human Behavior is seeking a compassionate and highly organized part-time Care Coordinator to support TIES for Families, a nationally recognized interdisciplinary program dedicated to improving outcomes for children with complex needs adopted from foster care. Working closely with the Director of Clinical Services and a multidisciplinary team, you'll play a key role in coordinating care, supporting families, and ensure services are delivered seamlessly.
Primary responsibilities include:
* Coordinate services and referrals for children and families, connecting them with internal and community-based resources
* Serve as a point of contact for families, providing guidance, support, and advocacy
