GENERAL JOB DESCRIPTION:

Under the general direction of the Program Director, the Care Coordinator will provide Care Coordination services to youth who have a mental health diagnosis that affects their daily functioning. The Care Coordinator will work with youth up to age 21 who reside in Monroe County who do not qualify for Medicaid.
