Cardiac Rehabilitation Practitioner (Band 6)

Make a meaningful difference to the lives of people living with heart failure by delivering high‑quality, evidence‑based cardiac rehabilitation services across community and clinical settings.

You will lead and support Phase 3 heart‑failure rehabilitation classes, work directly with patients in their own homes and contribute to optimal symptom management and the maximisation of medical therapy. Your role includes specialist assessment, advice, rehabilitation and ongoing support, ensuring care aligns with NICE guidance, BACPR standards, and local policies.
