Position Overview

Behavioral Health Case Manager

Reports to: Behavioral Health Director

Job Status: Full Time | FLSA Status: Non-Exempt | Travel: up to 50%

Work Schedule: 8:00 a.m. – 5:30 p.m., Monday–Thursday; 8:00 a.m. – 12:00 p.m. on Friday; 8:00 a.m. – 4:30 p.m. (UAM Clinic and other school‑based clinics). Adjustments or additional hours may be required.

Position Summary

The Behavioral Health Case Manager provides administrative support to the Behavioral Health Unit and to integrated and/or behavioral health patients, assisting in achieving program goals. Responsibilities include clerical tasks, patient education, referral management, care coordination, quality improvement activities, and participation in the patient‑centered medical home model of care.
