Position Overview
General Summary: Collaborating closely with supervising physicians, the Advanced Practice Clinician (APC) delivers direct patient care following established healthcare policies and protocols. This role involves comprehensive medical management of patients across a wide age range within a high-risk population. Responsibilities include diagnosing and managing medical conditions while coordinating with community partners to ensure integrated care.
Essential Functions: Conduct initial and periodic histories and physical exams. Provide and coordinate comprehensive ambulatory care based on education and training. Manage acute, episodic, and chronic conditions, referring patients when cases exceed scope of practice. Screen patients for further healthcare needs.
