An excellent opportunity has become available for a highly motivated, flexible and passionate Advanced Clinical Practitioner to join the Older Adult Home Treatment teams. People currently undergoing ACP training are eligible to apply for band 7 and people who have completed the training are eligible to apply for band 8a.

Currently there are two Home Treatment Teams within the Older Adult Service that the post holder will work across:

• Croydon Home Treatment Team based at the Bethlem Royal Hospital.
• Lambeth, Southwark and Lewisham (LSL) Home Treatment Team based at the Maudsley Hospital.

The post holder will support the clinical team leader in providing clinical leadership and managerial oversight for the allocated caseload, and will support transfers across the Older Adult pathways, taking a lead in clinical decision‑making.
