Position Description

Reporting to the Manager, Transition Management, the Transition Facilitator is accountable for coordinating seamless transitions across inpatient areas to the community. With a focus on excellence throughout the transition process, the facilitator provides guidance and mentorship to create efficient and effective transitions.
