Diabetes Wellness Program – Registered Nurse

Reporting Relationship

Reports to and works under the direction and supervision of the Clinical Lead Supervisor for the Diabetes Wellness Program and Primary Care Services, Six Nations of the Grand River Elected Council.

Job Summary

The role of the registered nurse is to provide supportive and collaborative chronic disease management services in the context of diabetes. These services can be one‑on‑one or in a group setting, provided in the environment that best addresses the needs of the individuals seeking services.

Position Details

• Type: Full‑Time
• Hours of Work: 35 hours per week
• Wage: $70,400 – $88,000 per year

Basic Qualifications

• Graduate of a Bachelor of Science in Nursing or
